i think i would prefer the first option most, personally. it implies a cure, somewhere.
yeah, saliva or blood in an open wound would do it, same as any other bloodborne illness. Blood transfusion would probably do it too. And you would THINK that being a curse would be the easiest one, just have to apologize and atone to whoever cursed you, but that presumes you're dealing with someone reasonable, and reasonable people don't go around cursing folks with lycanthropy, in my experience. Personally I prefer the hereditary option, means you can't pass it on by accident that way.
